# Therapy for Phobias and Specific Fears

A specific fear can be very small on paper and very large in daily life. Therapy can help you approach it gently, on your terms.

## What a Specific Fear Looks Like

A phobia is an intense fear of a particular thing or situation, strong enough that you go out of your way to avoid it. Common examples include needles, dogs, storms, driving, heights, flying, vomiting, insects and enclosed spaces. You may know perfectly well that the fear is bigger than the danger. Knowing does not make the fear go away.

Specific fears can start after a frightening experience, can be learned from watching someone else be afraid, or can seem to come from nowhere. Where it came from is worth understanding, but you do not need a clear origin story to work on it.

## How Avoidance Shrinks a Life

Avoidance is the engine of a phobia. Every time you avoid the feared thing, you feel relief, and your brain learns that avoiding it kept you safe. The fear gets a little stronger and the avoidance a little wider.

A fear of needles can mean putting off medical care. A fear of driving can mean depending on others for every errand. A fear of dogs can mean crossing the street, skipping friends' homes, or never going to the park. A fear of storms can mean checking the forecast constantly and feeling on edge through every rainy season. The cost of avoidance is often much larger than the feared thing itself.

## Gradual Exposure, Paced and Chosen With You

The most direct way through a specific fear is gradual exposure: approaching the feared thing in small steps, staying with each step until the fear begins to settle, and then moving to the next. This comes from cognitive behavioral therapy (https://ubuntucounselingeugene.com/approach/cognitive-behavioral-therapy), and it works by giving your brain new evidence.

What matters to me is how it is done. Exposure is never forced and never sprung on you. We build the ladder together, starting with something you rate as manageable, maybe looking at a picture of a dog, or sitting in the driver's seat of a parked car. You decide when to take the next step and when to stay where you are for a while.

Acceptance and commitment therapy (https://ubuntucounselingeugene.com/approach/acceptance-and-commitment-therapy) adds another piece: making room for the fear instead of waiting for it to disappear, and connecting each step to something you care about, like being able to take your child to the doctor or drive to see a friend.

## When the Fear Belongs to a Child

Children's fears are common and many pass on their own. When a fear starts shaping family life, such as bedtimes built around storms, outings canceled because of dogs, or medical visits that end in tears for everyone, therapy can help.

With children, exposure looks like play and curiosity. Parents are an important part of the work, because how the adults respond to a child's fear can either shrink it or accidentally feed it. I will help you find a way to be supportive without rearranging everything around the fear. My families and parents page (https://ubuntucounselingeugene.com/who-i-help/families-and-parents) says more about working together.

## Sensory Considerations

Some fears are tangled with sensory experience: the loud bark of a dog, the sting of a needle, the crack of thunder, the feeling of a crowded elevator. For autistic people and others with sensory sensitivities, part of what feels unbearable may be the sensory load itself rather than danger.

That changes the plan. We might work on sensory supports, such as headphones, pressure, fidgets or planning for a quiet exit, alongside gradual steps. Respecting sensory needs is not avoidance. It is making the steps possible. The office is a low arousal room with natural light and low noise, which helps.

## Where This Fits

Specific fears are one part of the anxiety work I do. The main anxiety therapy page (https://ubuntucounselingeugene.com/services/anxiety) describes the wider picture. Sessions are available in person in Eugene or by telehealth for anyone located in Oregon, and some steps are easier to practice from home.

## Questions About This Approach

Will you make me face my fear right away?

No. We start with a step you choose as manageable and build from there at your pace. Nothing is forced or sprung on you.

My child is afraid of dogs. Is that worth therapy?

Many childhood fears pass on their own. If the fear is shaping family life, therapy can help, and parents are part of the work. See my families and parents page (https://ubuntucounselingeugene.com/who-i-help/families-and-parents).

Can phobia work happen by telehealth?

Some of it can, for anyone located in Oregon. Early steps like images or imagining often work well on video, and in-person sessions are available in Eugene.
